Your submission is not complete unless you complete all three steps:

1. Fill out this Google Form for each piece - https://forms.gle/Jt8YRdbnPe325bNj9 
2. Include the completed paper form/slip securely attached to the art piece (for visual art, photography or literature) and drop off at McAuliffe Room 84 (LAEF office).
